Order a Poem -- If you'd like to order a poem, please click here to call up an order form. You will be asked to include the following information:

  • Your Name, Email Address, and Shipping Address
  • Child's Name (If child's name is 4 or less letters, include a middle name.)
  • Child's Gender
  • Child's Age
  • Whether or not you would you like religious references.
  • Middle name
  • Special interests or talents

    • Names by Nicole will attempt to personalize the poem by including special interests and talents insofar as they work within the poem. Please include no more than three, and state them in the order of importance.

    If you wish to have a photo or clip-art image printed on the poem, please be aware that you will need to supply a digital copy of the image you wish to use.

    Names by Nicole accepts payments via Paypal, Yahoo! PayDirect, Money Orders, Cashier's Check or Personal Check. Once your order is placed, you will receive a response E-mail with a price quote and directions for payments. Once the poem is complete, it will be sent out as soon as payment is received.
